Thymosin peptides: what the class is

Thymosin alpha-1 and thymosin beta-4 share a name, not a structure. TB-500 is a fragment of one of them. BPC-157 is not a thymosin at all.

Why we wrote this. Readers meet BPC-157 and TB-500 as one class. Only one of them is a thymosin, and the family member with real approvals is the one nobody is selling.

The word thymosin does not name a chemical family. It names a place where a set of peptides was first found. A calf thymus extract called thymosin fraction 5 was separated into its components, and thymosin alpha-1, polypeptide beta-1 and thymosin beta-4 came out of that separation[1]. Those peptides are not structural relatives. That one historical label is why TB-500 and BPC-157 land in the same forum thread, the same vendor bundle and the same search query, even though only one of them is a thymosin at all.

One label, two unrelated molecules

Thymosin alpha-1 is 28 residues long. UniProt records it as residues 2 to 29 of prothymosin alpha, a 111-residue protein[2]. Thymosin beta-4 is a separate gene product, 44 residues as translated, with the mature chain running from residue 2 to residue 44[3]. Different genes, different lengths, different jobs. The beta-thymosins are the main intracellular G-actin sequestering peptides in most vertebrate cells, which is a structural role inside the cytoskeleton[1]. Thymosin alpha-1 works on the immune system instead. Anyone reading a claim about thymosins as a group should ask which one, because the answer changes the pharmacology completely.

BPC-157 is not a thymosin

This is the correction the class page exists to make. BPC-157 is a pentadecapeptide, 15 amino acids, and the Zagreb research group behind most of its literature describes it as stable and native in human gastric juice[4]. It comes from a gastric protein, Body Protection Compound, which is where the initials come from. It shares no sequence, no parent protein and no origin story with thymosin alpha-1 or thymosin beta-4. It is grouped with TB-500 because the two are sold into the same injury-recovery market and are frequently taken together, not because they share biochemistry. The family member with real approvals is the one nobody markets

Thymosin alpha-1, sold as Zadaxin and known by the international non-proprietary name thymalfasin, is the outlier. Goldstein and Goldstein wrote in 2009 that it is approved in over 35 countries for the treatment of hepatitis B and C, and as an immune stimulant and adjuvant, and that it was then in late-stage clinical testing in the United States and Europe for hepatitis C and stage IV melanoma[5]. Registered trial activity matches that picture. ClinicalTrials.gov returns 65 registered studies for thymalfasin, spanning COVID-19 vaccine response in older adults, non-small-cell lung cancer and pancreatic cancer[6]. That is what a peptide looks like when sponsors have taken it through regulatory systems. Neither BPC-157 nor TB-500 has an approval anywhere in our coverage area, and neither has been approved by the FDA or the EMA.

TB-500 is a fragment, and the trials are for the whole molecule

Thymosin beta-4 is the major actin-sequestering molecule in eukaryotic cells, with a documented role in dermal and corneal wound healing[7]. TB-500 is not that molecule. Doping-control chemists analysed a TB-500 product in 2012 and identified in it the N-terminally acetylated 17 to 23 fragment of human thymosin beta-4, Ac-LKKTETQ[8]. That heptapeptide sits inside the full sequence[3] and covers the actin-binding stretch, but a fragment is not the parent peptide, and the published human work used the parent. ClinicalTrials.gov returns 18 registered studies for thymosin beta 4[9], and the first ones listed are the RGN-259 ophthalmic programme (ARISE-1, ARISE-2 and ARISE-3 in dry eye, SEER-2 in neurotrophic keratopathy) plus a recombinant thymosin beta-4 injection in acute myocardial infarction. Eye drops and cardiology, not tendons. The same database returns 2 registered studies for BPC-157[10].

Both are prohibited in sport, under different sections

The doping status splits along the same line as the biochemistry. USADA's summary of the 2018 Prohibited List changes records that thymosin-beta4 and its derivatives, for example TB-500, were added as examples of prohibited growth factors under S2.3[11], effective 1 January 2018. BPC-157 sits somewhere else entirely, in S0, the non-approved substances category, and the US Department of Defense Operation Supplement Safety programme states that it is an unapproved drug and cannot be legally prescribed or sold over the counter[12]. What we don't yet know

No published trial has compared these compounds head to head in one protocol. For TB-500 specifically, we do not know whether the acetylated heptapeptide reproduces in people what the full-length parent does, because the registered human programme has used the parent peptide and has run it as an eye drop rather than a systemic injection. For BPC-157, the registered human record is two studies, one of which is a phase 2 hamstring-strain trial that has not reported. Long-term safety across months of continuous use has not been characterised for either in any pharmacovigilance system in our coverage area, and the identity problem is real: without a batch certificate of analysis, what is in a grey-market vial is an assumption. Thymosin alpha-1 has the deepest human record of the three, and it is also the one almost nobody in the injury-recovery market is actually buying.

Where this leaves you

Read the label sceptically. Thymosin alpha-1 and thymosin beta-4 share a name because of a 1970s calf thymus extract, not because they are chemical cousins. TB-500 is a seven-residue piece of thymosin beta-4 rather than the peptide the trials used. BPC-157 is not in the family at all and only travels with it commercially.
