Tirzepatide and Sleep Apnea Endotypes
A single SURMOUNT-OSA participant had major changes in sleep apnea and airway physiology after tirzepatide, but one case cannot establish mechanism.
Why we wrote this. The detailed sleep-lab measurements are unusual and informative, but a one-person report must not be mistaken for a treatment-effect estimate.

A case report followed one 35-year-old man with obesity and severe obstructive sleep apnea during 12 months of tirzepatide treatment in the SURMOUNT-OSA trial. His body mass index fell from 31.0 to 24.9 kg/m². In the report's sleep-study table, the apnea-hypopnea index fell from 56.3 to 4.4 events per hour, moving from the severe range to below the usual diagnostic threshold[1]. The detailed physiology is interesting. It remains one person's course and cannot show which changes tirzepatide caused.
What happened over 12 months
The participant presented with loud snoring and nasal obstruction. He had poorly controlled hypertension, allergic rhinitis, a redundant soft palate, and an elongated uvula. A diagnostic sleep study in December 2022 recorded an apnea-hypopnea index of 54.8 events per hour. The later analysis table used a baseline value of 56.3, apparently reflecting the recording selected for the endotype comparison. He received tirzepatide during 2023 and underwent follow-up laboratory sleep studies at six and 12 months[2].
At 12 months, waist circumference had fallen from 105 to 93.3 cm. The lowest overnight oxygen saturation increased from 68% to 88%, and the share of sleep time below 90% oxygen saturation fell from 11.1% to 0.1%. Hypoxic burden, a measure combining the depth and duration of oxygen drops, declined from 113.1 to 20.6 percentage-minutes per hour. Sleep efficiency increased from 85.2% to 94.1%[2]. These are repeated measurements in the same individual, not a controlled comparison.
This individual was drawn from SURMOUNT-OSA, but the case report serves a different purpose from the parent trial. The paper notes that the larger trial found an average reduction of 25.3 apnea-hypopnea events per hour and 17.7% body-weight loss at week 52, with more than 70% of participants improving their apnea-hypopnea index by over half[2]. Those group results provide efficacy context. The case report adds detailed physiology from one participant, whose roughly 92% index reduction was larger than the trial average. Selecting an unusual response for deeper analysis can illuminate a mechanism, but it cannot estimate how common that response is.
What the endotype measurements mean
An endotype is a physiological pathway that can contribute to the same outward diagnosis. Obstructive sleep apnea can reflect a collapsible upper airway, unstable breathing control, weak muscle compensation, or a tendency to wake at a particular level of breathing disturbance. The authors derived these traits from non-rapid-eye-movement sleep using a polysomnography-based method. This goes beyond counting breathing interruptions and asks how they may be arising.
Passive airway ventilation increased from 70.6% to 95.3% of normal breathing, which the authors interpreted as better airway patency. Loop gain fell from 0.51 to 0.30, suggesting more stable ventilatory control. Muscle compensation changed from -33.5% to 10.7%, while active ventilation rose above passive ventilation by month 12. The authors read that pattern as stronger recruitment of upper-airway muscles during obstruction[2]. Those interpretations are mechanistic hypotheses generated from one data series.
The unexpected event-duration finding
Not every measure moved in an intuitively favorable direction. Average apnea duration increased from 26.3 seconds at baseline to 36.9 seconds at month 12, while average hypopnea duration increased from 29.7 to 60.4 seconds. The arousal threshold fell, yet respiratory events became longer[2]. The authors propose that improved muscle recruitment may have ended events with fewer arousals. That explanation is plausible within their model, but the case cannot rule out measurement variability or another explanation.
The report states that no adverse events were observed in this participant during treatment[2]. A single person with no reported event provides almost no information about population safety. Readers should use larger randomized trials and authorized product information for that question. The tirzepatide safety overview summarizes the broader evidence.
What we don't yet know
The participant lost substantial weight while several sleep and airway measures changed. The case cannot separate effects related to weight loss from any direct effect of tirzepatide on breathing control or airway muscle tone. There was no untreated comparison, no repeated baseline period, and no way to estimate how often this endotype pattern occurs. The authors themselves call for further mechanistic work[1]. A case report is best used to form a testable question, not settle it.
Two authors were employees of Nox Medical, whose research tools were used for the physiological analysis. Other authors reported no conflicts, and two investigators received public research funding from Taiwan's National Science and Technology Council. The paper says the funders had no role in the work or publication decision[2]. These disclosures matter when reading a technically specialized analysis.
This case does not support stopping positive airway pressure or changing sleep-apnea care without medical supervision. A person can feel better while still having clinically important overnight breathing problems. Follow-up testing and treatment decisions belong with a qualified sleep clinician. Frequently asked
How much did sleep apnea change in this case?
The report's comparison table shows the apnea-hypopnea index falling from 56.3 to 4.4 events per hour over 12 months. This was one participant and should not be treated as an expected result.
What is a sleep apnea endotype?
It is an underlying physiological trait that contributes to obstructive sleep apnea, such as airway collapsibility, unstable breathing control, muscle compensation, or the threshold for waking from sleep.
Did the case prove tirzepatide directly improves airway function?
No. Airway and breathing-control measures changed alongside substantial weight loss, but one uncontrolled case cannot separate a direct drug effect from weight-related changes or measurement variability.
Can sleep apnea treatment be stopped after weight loss?
Not on the basis of this case. Sleep apnea severity requires clinical reassessment, usually including follow-up sleep testing. Treatment changes should be made with a qualified sleep clinician.
